Liability Waiver and Indemnification

I understand that Roswell Community Masjid, ILM Academy or Nur ul Quran cannot assume responsibility for the safety and welfare of attendees while attending the Roswell Community Masjid, ILM Academy or Nur ul Quran event and engaging in the various games, activities, including but not limited to taking part in the petting zoo and the inflatable obstacle course. I understand that my child is my responsibility and must abide by all Roswell Community Masjid, ILM Academy or Nur ul Quran regulations and volunteer instructions while taking part in the activities mentioned above. I understand that Roswell Community Masjid, ILM Academy or Nur ul Quran volunteers cannot prevent injuries because they cannot always control the condition present or be present at all times.

Filling out this form constitutes and is evidence of my agreement to everything above in addition to: 

(1) accepting general liability for the attendance and taking part in the activities by my child(ren) and family members at the event.

(2) indemnifying and holding Roswell Community Masjid, ILM Academy or Nur ul Quran, its employees, volunteers and agents, either jointly or severally, from and against any and all claims, damages, causes of action or injuries, including reasonable attorney’s fees and costs expended in defense there of, incurred or resulting from your child(ren) and family's participation in the fall picnic and all related activities at the event.
